Attending Physician(s) remain as the patient's primary care physician and work as a member of our hospice team.

Certifies the attending physician’s prognosis; leads the hospice team in developing a plan of care; provides consultation to attending physician.

Evaluates the patient’s condition during regular schedule visits. Coordinates the plan of care techniques with the family including patient care techniques and use of medications.

Licensed medical social workers provide emotional support for the patient and family, meanwhile counseling in dealing with end-of-life issues and assisting with advanced directives.
